Feedback form

If you would like to make a comment or a formal complaint about Epilepsy Action or any of its work, please can either do so online, or by downloading our paper feedback form.

If you wish to include other information, including photographs or leaflets, then this can't be done online, so please download the feedback form and send in your feedback by post. If the spaces in the form do not provide enough space for you to provide full details, please use a separate sheet of paper. When you have completed the form please send it, together with any other paperwork, to the address given at the bottom of page 6.
