Gift Aid Declaration

I, the below named, declare that all donations, including subscriptions I make to Epilepsy Action and all those made in the last 4 years are to be treated as Gift Aid donations.

I am a UK tax payer and I pay an amount of Income Tax or Capital Gains Tax equal to or greater than the amount Epilepsy Action will reclaim on my donation in the tax year (25p for every £1 given). 

(This may be tax paid from your wages, pensions or bank and building society interest.)
