Information in Wales about long term medical conditions

The Long Term Conditions Alliance in the Welsh Assembly Government is surveying people with long term medical conditions.  They want  to find out what information people use about their condition, how they use it and what information is missing.

If you live in Wales, please respond to their survey.  Please download it, fill it in and send it to the address on its second page.

The closing date is Friday 9 April
