Southern Health Trust fined £2m over breaching health and safety laws in cases of Connor Sparrowhawk and Teresa Colvin

Southern Health NHS Foundation Trust has been fined £2m following the preventable deaths of two patients in its care.

Teresa Colvin and Connor Sparrowhawk both died at facilities run by the trust in 2012 and 2013. Connor, who was 18, drowned in the bath at Slade House in Oxford, following an epileptic seizure.

The doctor in charge of the care of Connor Sparrowhawk who drowned in the bath after having a seizure in 2013, has been suspended.

Dr Valerie Murphy was facing a medical practitioners tribunal to determine her fitness to continue to practise medicine. This is after it was found that she made a number of failings in Connor’s care during this time.

Dr Valerie Murphy’s fitness to practise medicine has been found to be impaired by misconduct. This was decided by a medical practitioners tribunal on Sunday 12 October.

Dr Murphy was in charge of the care of 18-year-old Connor Sparrowhawk at an NHS facility in 2013. Connor, who had epilepsy, learning disabilities and autism, died after having a seizure in the bath.

Southern Health Trust pleads guilty in the case of Connor Sparrowhawk

Southern Health NHS Foundation Trust has pleaded guilty to breaching health and safety law in the case of Connor Sparrowhawk who died at a facility ran by the trust in 2013.

A representative of the trust attended Banbury Magistrates Court on Monday 18 September for the criminal prosecution hearing brought forward by the Health and Safety Executive (HSE).
